Hudson-Essex Nephrology is the largest premier of private practice physicians specializing in the field of Nephrology in the Hudson/Essex, New Jersey area. We provide superior care to individuals with renal (kidney) disease. As experts in the treatment of Chronic Kidney Disease (CKD), End Stage Renal Disease (ESRD), Acute Renal Failure (ARF), kidney transplantation maintenance, and creating and maintaining accesses for dialysis therapies - we are pleased that you have chosen us for your Nephrology healthcare needs.

Hudson-Essex Nephrology has enjoyed the trust of patients and the respect of the local communities in the New Jersey area for over 35 years. In order to minimize the time needed for you appointment, there are two very important forms that require completion: the “Welcome to Our Practice” and the “Health Questionnaire” forms. While these forms are lengthy, we ask that you take your time to thoroughly complete both forms. visit or brought with you on your appointment day. These forms can either be mailed prior to your visit or brought with you on your appointment day.

Please do not hesitate to call our office if we can be of any assistance to you in completing the required forms or in obtaining your medical records/test results. As our patient, it is your responsibility to keep all of your follow-up appointments. If, at any time, you are unable to keep an appointment, we ask that you call the office - at least 24 hours in advance - to cancel.

The following is a handy check-off list of the items you need to bring:


    1. The completed Welcome to Our Practice and Health Questionnaire.
    2. Diagnostic and Blood test results.
    3. ALL current medication bottles, including over-the-counter medications. Medications are a very important part of your health assessment, so please bring the actual bottles of medication.
    4. Current insurance ID card(s).
    5. Written request for a consultation or Referral Form by your Primary Care Physician.
    6. Current photo ID.
    7. Co-payment (if required by insurance carrier).

        Please do not hesitate to call our office if we can be any assistance to you in either completing the enclosed forms or in obtaining your test results. As our patient it is your responsibility to keep all of your follow-up appointments. If at any time you cannot keep an appointment, we ask that you call and cancel as soon as possible. This will allow another patient to use the time.

        Every effort is made to keep waiting time in the office to a minimum. However, emergencies arise, patients arrive late, and some patients need more time than others. If we do fall behind, we hope you will understand. Please remember that you could be the patient who requires extra attention one day.
